Position Overview:
We are seeking an experienced Primary Care Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join a collaborative, team-based primary care model. This role focuses exclusively on comprehensive primary care for an established patient population, including preventive care, acute visits, and chronic disease management.
The APP functions as a licensed, independent provider in accordance with New York State regulations and organizational protocols, exercising sound clinical judgment and evidence-based practice. This position does not include specialty or surgical services.
Primary Job Duties:
Clinical Care

Provide comprehensive primary care services including preventive care, wellness visits, acute illness evaluation, and chronic disease management

Perform complete and focused histories and physical examinations

Diagnose and manage common primary care conditions

Prescribe medications, therapies, and treatments within scope of practice

Order, interpret, and follow up on labs, imaging, and diagnostic tests

Develop and monitor individualized care plans

Emphasize preventive care, lifestyle modification, and patient education
Job Responsibilities
Educate patients and families on diagnoses, treatment plans, and preventive care
Collaborate with physicians, nurses, medical assistants, and care coordinators
Participate in daily huddles and interdisciplinary care planning
Coordinate referrals and follow-up care as needed
Documentation & Quality
Document patient encounters accurately in the EMR
Utilize population health tools to support quality metrics and outcomes
Follow clinical guidelines, documentation, and billing standards
Professional Practice
Maintain current knowledge of primary care guidelines
Participate in CME, provider meetings, and quality improvement initiatives
Practice within a patient-centered medical home and team-based care model
Qualifications
Board certified in Family Medicine or Internal Medicine
Valid and unrestricted New York State license
Valid DEA license (or ability to obtain)
Current BLS/ACLS certification
Clean malpractice history
Strong clinical, communication, and teamwork skills
