Nurse Practitioner – Annual Health Assessments

Salem, Oregon (Clinic + In-Home within 30-Mile Radius)
Full-Time | M–F | 8:00 AM–5:00 PM PST

Position Overview

  • Permanent opportunity due to organizational growth

  • Primary Duties:

  • Perform Annual Wellness Exams and health risk assessments for senior Medicare patients

  • Deliver care in clinic settings and in-home visits

  • Focus on high-risk patients, chronic condition management, and post-discharge support

  • Collaborate with interdisciplinary team (IDT) — social workers, case managers, etc.

  • Perform HCC coding, care coordination, counseling, screenings, and discharge planning

  • Expected patient volume:

  • 10–12/day in clinic

  • 5–6/day in-home

Charting due within 72 hours

Ideal Candidate

Passionate about serving underserved/Medicare populations

Strong commitment to preventative, holistic, value-based care

Excellent communication and patient education skills

Comfortable with both clinic and home visits

Qualifications

Current Oregon NP license

FNP, Adult, Gerontology, or Palliative Care certification (ANCC/AANP)

2+ years of NP experience in primary care or similar fields

Home health or acute care experience a plus

DEA License + Prescriptive Authority (can be reimbursed if obtained post-hire)

CPR/BLS Certification

Valid Oregon Driver’s License

EMR proficiency (client uses their own internal system)

Compensation & Benefits

Annual Salary: $125,000 – $150,000 (based on experience)
