Seeking a full-time Nurse Practitioner to join its interdisciplinary PACE team. This role focuses on delivering comprehensive, community-based medical care to frail and elderly adults, supporting independence, dignity, and quality of life while enabling participants to remain safely in their homes and communities.
Job Responsibilities
• Develop and implement individualized care plans
• Evaluate and treat acute illnesses
• Participate in discharge planning
• Conduct home visits as clinically indicated
• Coordinate care across the full continuum of PACE services
• Assist in development and review of clinical policies and procedures
• Participate in provider on-call rotation
Qualifications
• Active Massachusetts NP license – Required
• DEA Registration – Required
• CDS Certification – Required
• BLS Certification – Required
• Recent NPDB report (within 30–60 days) – Required
• Experience working with frail or elderly populations – Required
• Ability to travel within the service area – Required
Working Hours
• Monday–Friday (PACE program hours)
• On-call rotation participation required
