Position Summary

The organization is seeking a Primary Care Nurse Practitioner (APRN) to join its established outpatient practice in Southbury, Connecticut. Candidates with a background in Family Medicine or Internal Medicine–focused primary care will be considered.

This is a full-time, permanent position providing comprehensive primary care services to adult and geriatric patients in an outpatient setting. New graduates with strong clinical training and experienced providers are encouraged to apply.

Essential Responsibilities

Provide comprehensive primary care services, including preventive care, chronic disease management, and acute visits
Diagnose, treat, and manage common medical conditions across the adult patient population
Conduct annual physicals, wellness visits, and health risk assessments
Order and interpret diagnostic tests and laboratory results
Collaborate with physicians, specialists, and interdisciplinary care teams
Maintain accurate and timely documentation in the EMR
Participate in quality improvement initiatives and evidence-based care delivery
Educate patients on disease prevention, treatment plans, and healthy lifestyle choices

Qualifications

Education: Master’s or Doctoral degree in Nursing from an accredited program
Licensure: Active Connecticut APRN license (or eligibility)
Certification: Board Certified as FNP or Adult/Gerontology NP
Training Focus: Family Medicine or Internal Medicine–based primary care
Experience: All experience levels welcome; new graduates encouraged to apply
