Responsibilities
• Provide comprehensive outpatient primary care
• Manage chronic conditions and preventive services
• Educate patients on wellness and lifestyle management
• Document care accurately in the EMR
Requirements
• Nurse Practitioner or Physician Assistant
• Primary care experience required
• Active state license (or ability to obtain)
