Nurse Practitioner (NP) – Long-Term Care

Location: Pennsylvania
Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM (40 hours/week)

Job Summary

We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality primary and preventive care to residents in skilled nursing facilities. This role focuses on comprehensive medical management, advanced care planning, and collaboration with interdisciplinary care teams to improve outcomes for patients with complex and chronic conditions.

Responsibilities

  • Provide primary and preventive medical care to long-term care residents in skilled nursing facilities

  • Manage residents experiencing acute changes in condition

  • Perform advanced care planning and chronic care management

  • Prescribe medications, including controlled substances, in accordance with Pennsylvania law

  • Order, review, and follow up on diagnostic tests and treatment plans

  • Communicate regularly with nursing home staff regarding patient status and care plans

  • Collaborate closely with primary care physicians and interdisciplinary teams

  • Maintain accurate and timely documentation in the electronic medical record (EMR)

Required Qualifications

  • Current, unrestricted Nurse Practitioner license in the state of Pennsylvania

  • Board Certified Nurse Practitioner

  • Active DEA license

  • Clinical background in Adult, Family, or Geriatric specialties

  • Experience working in a long-term care or skilled nursing setting

  • Proficiency with electronic medical records (EMR)

  • Ability to manage patients with advanced illness and multiple chronic conditions

Preferred Qualifications

  • Experience in geriatrics or post-acute care

  • Strong communication and care coordination skills

  • Comfort working independently in a facility-based setting

Schedule & Employment Type

  • Monday–Friday, 8:00 AM – 5:00 PM

  • 40 hours per week

  • Full-Time (5 days/week)
