Nurse Practitioner – Adult / Geriatric Primary Care

Location: Mishawaka, Indiana
Schedule: Full-Time | Outpatient | Monday–Friday

Outpatient Primary Care – Adult & Senior Population

A well-established outpatient primary care practice in Mishawaka, IN is seeking a Nurse Practitioner (NP) to join a team-based, value-focused care environment serving adults and seniors.

This role is ideal for Nurse Practitioners who value longer visit times, continuity of care, and managing complex patients within a supportive, physician-led clinical model.

Position Highlights

  • Outpatient primary care only

  • Monday–Friday schedule

  • No hospital rounding or inpatient responsibilities

  • Focus on adult and geriatric patients

  • Team-based care with strong clinical and administrative support

  • Emphasis on quality, outcomes, and preventive care

Key Responsibilities

  • Provide comprehensive primary care services for adult and senior patients

  • Diagnose, treat, and manage acute and chronic conditions

  • Deliver preventive care and wellness services

  • Collaborate closely with physicians and interdisciplinary care teams

  • Practice within established clinical guidelines and quality measures

  • Participate in population health and value-based care initiatives

Qualifications

  • Certified Nurse Practitioner (NP)

  • Active or eligible Indiana NP license

  • Prior primary care or adult/geriatric experience preferred

  • Comfortable managing patients with multiple chronic conditions

  • Interest in preventive and value-based care models
