- Travel to each assigned nursing facility weekly (up to 65 mile radius)
- Assess patient and make recommendations on plan of care
- Work closely with nursing staff and facility administration
- Utilize facility formulary and follow facility protocols
- Document patient encounter in our EHR (at time of service)to ensure timely reporting
- Discuss treatments and goals with PCP and family members
- Procedures – Debridement’s, I & D’s, Cauterization, NPWT, Skin Substitutes, Biopsies, Etc
