Job Details

Responsibilities and Duties
- Provide advanced wound care treatment to patients with wounds unhealed after 30+ days of conservative management
- Conduct initial assessments and develop individualized care plans
- Utilize amniotic membrane grafts and amniography as primary treatment modalities
- Order and apply synthetic skin/skin grafts as indicated
- Perform wound debridement and rewrapping during each visit
- Follow care plans and operate autonomously within the patient’s home
Additional Information
- Indiana Nurse Practitioner license required
- Wound Care board certification required
- Advanced wound care experience preferred
- Must be comfortable working independently in a home care setting
