Family Nurse Practitioner - Albuquerque, NM
Shift: Full-Time (Mon – Fri)

Key Responsibilities
• Conduct patient assessments, diagnose conditions, and develop treatment plans
• Provide primary and urgent care services based on specialty
• Manage acute and chronic medical conditions
• Prescribe medications and order diagnostic tests
• Maintain accurate documentation in EHR/EMR systems
• Collaborate with physicians and interdisciplinary care teams
• Provide patient education, preventive care, and health counseling
• Participate in telehealth services and care coordination
Requirements
• Active Nurse Practitioner license with current state certification
• Valid DEA license
• Ability to work independently and as part of a multidisciplinary team
• Experience with EHR systems and clinical documentation
• Strong communication and patient care skills
• Experience in primary care, geriatrics, behavioral health, or chronic disease management preferred
