Advanced Practice Provider (FNP or PA) | Salary: $104,019–$139,792

Requirements to apply

• North Carolina license (PA or NP)
• DEA certification
• Basic Life Support certification
• Completion of an approved NP/PA program
• Medical experience with geriatric/high-risk populations

Full-time or part-time opportunities supporting senior care within a PACE program.
Mission-driven clinical role providing comprehensive primary care services to an aging population Monday–Friday, 8am–5pm.

Job Overview

We’re seeking an Advanced Practice Provider to deliver the full scope of ambulatory primary care to geriatric participants within an integrated PACE model. The ideal candidate is compassionate, collaborative, and comfortable managing a broad range of clinical needs within their scope. Responsibilities include patient visits, telephone consultation, medical documentation, participation in quality initiatives, and coordination with multidisciplinary team members. Complex cases receive physician support as needed.
