Job description We are seeking a Family Practice Nurse Practitioner for a locum tenens assignment in South Dakota. This role begins Jan 5, 2026, with a flexible schedule of 1-3 days per week and 8-hour shifts. The assignment is 1 week in length and focuses on functional medicine and hormone therapy in a dynamic, telehealth-driven environment.

Responsibilities and Duties
  • Conduct initial consultations and new patient lab reviews via video
  • Review and interpret advanced labs, including GI-MAP, Methyl Detox/Genomic Panels (MTHFR, COMT, AHCY), ALCAT/food sensitivity testing, and comprehensive hormone/metabolic panels
  • Develop personalized care plans, incorporating lifestyle, nutrition, supplementation, and prescription therapy as appropriate
  • Manage asynchronous follow-up care, including patient messaging, protocol updates, and lab result interpretation via EMR platform
  • Educate patients on test results and coach on health optimization
  • Complete all documentation in EMR (CHARM/CANVAS) within 24 hours
  • Provide functional medicine–informed SOAP notes and rationale for care plans
  • Communicate with support staff (RN, MA, pharmacy team) to coordinate patient care
  • Participate in chart audits or case reviews as requested by leadership
  • Prescribe and manage bioidentical hormones, peptides, nutraceuticals, and other therapeutic agents as appropriate
  • Monitor patient response, adjust dosing, and document medication changes
  • Collaborate with compounding pharmacies and support teams for medication delivery

Additional Information
  • Must have a base level of understanding with hormone therapy
  • Experience with functional medicine and hormone therapy preferred; candidates with varied backgrounds and experience levels will be considered
  • Drug Enforcement Administration (DEA) certification required
  • Board certification as an Adult-Gerontology Nurse Practitioner (A-GNP) or Family Nurse Practitioner (FNP) required
  • South Dakota licensure required
  • Flexibility, adaptability, and strong communication skills are essential
  • Training available for less experienced candidates who are adaptable and quick to learn
