Responsibilities and Duties
- Meet with residents and patients at contracted assisted living facilities to conduct initial patient visits
- Establish provider-patient relationships and perform physical assessments, including listening to lungs and heart
- Log patient interactions and findings into the OmniCare EMR system for tracking by the clinical team
- Travel to facilities within an hour from your home area as needed

- Willingness to travel up to an hour outside of your home area is required
