Annual Salary: $130k-$180k (varies depending on years of experience)
We are seeking a dedicated Family Nurse Practitioner with strong experience in community health and/or Federally Qualified Health Center (FQHC) settings to join our client’s patient-centered, mission-driven team. The ideal candidate is passionate about working with diverse, underserved populations and committed to improving access to high-quality primary care.
Responsibilities
- Provide comprehensive primary care across the lifespan, including health assessments, diagnosis, treatment, and management of acute and chronic conditions.
- Deliver culturally competent, trauma-informed, and patient-centered care tailored to community health needs.
- Conduct routine screenings, health maintenance visits, preventive care, and chronic disease management.
- Order, perform, and interpret diagnostic tests and labs; develop appropriate treatment plans.
- Prescribe medications according to state and federal guidelines.
- Provide education on disease prevention, medication adherence, lifestyle management, and self-care.
- Address social determinants of health and collaborate with support staff to connect patients to community resources.
- Participate in outreach efforts, health education initiatives, and quality improvement projects focused on community needs.
- Communicate effectively with diverse patient populations, including those with limited English proficiency, complex medical histories, or limited access to care.
- Work closely with physicians, behavioral health providers, care managers, and support staff as part of an interdisciplinary team.
- Participate in case conferences, team huddles, and care coordination activities to ensure continuity and effectiveness of care.
- Collaborate with external community organizations, public health entities, and social service agencies when appropriate.
- Maintain thorough, accurate, and timely documentation in the electronic health record (EHR).
- Adhere to all clinic protocols, state and federal regulations, and quality and safety standards.
- Participate in audits, compliance activities, and performance improvement initiatives.
Qualifications
- Current, unrestricted FNP license and certification.
- Master’s or Doctorate degree in Nursing from an accredited program.
- Minimum 1–2 years of relevant clinical experience in community health, primary care, or FQHC settings.
- Experience working with underserved, low-income, or high-need populations.
- Strong understanding of culturally competent care, trauma-informed practices, and health equity principles.
