Job Responsibilities

Responsibilities:
As a Primary Care Nurse Practitioner, you will serve as a practicing Nurse Practitioner responsible for the overall management of patients enrolled in healthcare services provided by the Tennessee Valley Health Care System. This includes both administrative and clinical responsibilities, ensuring the highest level of care for our Veterans.
Clinical Responsibilities:
• Medical Home Implementation: Fully implement components and processes related to the VHA Primary Care Medical Home (PCMH) healthcare system design.
• Patient Coordination: Coordinate care for a panel of outpatients as assigned by TVHS.
• Health Evaluation and Treatment: Evaluate and treat acute and chronic health problems for scheduled and unscheduled patients. Perform histories and physical exams.
• Documentation: Provide appropriate documentation including history, physical exam findings, diagnosis, definitive care plan, and patient disposition. Ensure appropriate disposition that clearly indicates the next steps of care.
• Medication Management: Prescribe medications as appropriate, provide patients with a list of current medications and an appointment list at close-out.
• Procedures and Testing: Perform office procedures within approved privileges, order routine laboratory, radiological, and diagnostic tests, provide initial interpretation of results, and communicate test results to patients.
• Patient Communication: Respond to patient inquiries, provide patient and family education, order immunizations and preventive or screening services, and screen patients for the need for hospitalization.
• Life Support: Perform Basic Life Support as necessary.
• Clinical Reporting: Communicate clinical findings to other providers using the Computerized Patient Record System.
Administrative Responsibilities:
• Clinic Operations: Handle administrative tasks associated with clinic operations.
• Leadership: Provide leadership and direction to support staff assigned to your Patient Aligned Care Team (PACT) teamlet.

The ideal candidate will possess proficiency in all aspects of general primary care practice. You should have the expertise and competency to:
• Offer consistent, courteous, and respectful communication and treatment to all customers, including veterans, their representatives, visitors, and VA staff.
• Provide consistent information according to established policies and procedures, and handle conflicts and problems constructively and appropriately.
