Position Summary
The facility is seeking a dedicated and compassionate
Family Nurse Practitioner
to join its growing primary care team in the
Bronx, New York
.
This full-time outpatient role offers a stable schedule, supportive clinical environment, manageable patient volumes, and access to experienced physicians, APPs, nursing staff, and care management resources. The FNP will provide comprehensive primary care services, support preventive health efforts, and contribute to coordinated care across the organization.

Essential Position Responsibilities
Primary Care Clinical Duties
Provide comprehensive primary care to patients across the lifespan
Perform routine exams, follow-ups, chronic disease management, and sick visits
Order and interpret diagnostic tests
Develop and implement patient-centered treatment plans
Ensure timely and accurate documentation in the EMR
Procedures & Clinical Skills
Perform basic outpatient procedures as appropriate for scope of practice
Manage acute and chronic conditions in compliance with clinical guidelines
Collaboration & Communication
Work closely with physicians, care coordinators, nurses, and support staff
Participate in team huddles, care planning, and interdisciplinary coordination
Engage in patient education and promote preventive health measures
Maintain clear and professional communication with patients and families
Practice & Quality Support
Participate in quality improvement initiatives and population health programs
Uphold organization standards for patient safety, compliance, and evidence-based care
Contribute to workflow efficiency and overall clinic performance
Qualifications
Education
Master's Degree in Nursing
Completion of an accredited Family Nurse Practitioner program
Licensure & Certification
Active New York State FNP License (required)
Board Certification as a Family Nurse Practitioner
Current BLS certification
Experience
Experience in primary care or outpatient practice preferred
New graduates with strong clinical rotations will also be considered
