Responsibilities

Provide patient assessments, diagnosis, treatment, and follow-up care

Perform hormone replacement therapy

Order and interpret labs/diagnostics; prescribe medications

Develop individualized treatment plans with physicians

Educate patients on prevention, management, and healthy lifestyles

Maintain accurate EMR documentation
Qualifications

NP (MSN/APRN) or PA (accredited program + NCCPA)
-
Must have an active Colorado license
-
3+ years NP/PA experience + Functional Medicine experience

Hormone therapy experience
-
Strong clinical, diagnostic, communication, and teamwork skills

EMR proficiency
