Nurse Practitioner – Geriatric, High-Risk Patients
Location: Greater Los Angeles area: Downtown, ELA, South LA (Downey, Southgate, Alhambra)
Job Details:

Type: Permanent

Setting: combination of dialysis centers, nephrology offices, and patient homes (within a 30-mile radius)

Patient Panel: 300–400 patients

Focus: Geriatric and high-risk population
-
Telehealth: None
-
EMR: Epic
-
Credentialing Timeline: ~2 months
Primary Responsibilities:
-
Deliver comprehensive, high-touch care to complex geriatric patients
-
Prevent hospital readmissions and reduce avoidable utilization
-
Conduct:
-
Annual Wellness Visits (AWVs)
-
Risk assessments
-
Medication management
-
Manage chronic conditions and coordinate specialty care
-
Participate in Interdisciplinary Team (IDT) meetings
-
Ensure accurate EMR documentation and HCC coding
Training & Support:
-
Onboarding: 3 weeks
-
Support Team: Includes MAs, MD advisor, GGH’s CMO, and clinical leaders
-
IDT Collaboration: With social workers, case managers, etc.
Ideal Candidate:
-
Experience: 2+ years as NP in Primary Care, Internal Medicine, Geriatrics, or Palliative Care
-
Skills: Knowledge of Medicare, chronic care, and complex care management
-
Bonus: Home health or acute care experience
-
Language: Spanish bilingual preferred
Compensation & Benefits:

Salary: $145,000 – $160,000 annually
