COMPENSATION: $130,000 BASE TOTAL Potential $143,000+ Full Benefits

Summary:
Optigy is partnering with a leading value-based care organization to hire compassionate and experienced Advanced Practice Providers to deliver high-quality, home-based primary care for a Medicare-aged population in Kansas City. Providers will deliver care directly in patients homes, reducing barriers to access and improving health outcomes for underserved populations.

Key Responsibilities:

• Deliver primary care and consultative services in members' homes

• Partner with leadership to identify and schedule patients in need of home visits

• Collaborate with Medical Assistants and care teams to ensure quality, continuity, and patient satisfaction

• Maintain accurate documentation of all services and encounters

• Participate in quality improvement activities and ongoing care planning

• Provide care within an assigned geographic area with reliable transportation

Qualifications:

• Active and unrestricted NP or PA license in Missouri

• BLS/ACLS certification

• Experience in geriatrics, internal medicine, or family medicine preferred

• Comfortable managing patients with multiple chronic conditions

• Ability to lift and transport up to 20 lbs. of equipment

• Strong communication, empathy, and clinical decision-making skills

• Reliable vehicle and valid drivers license required
