Home Health Nurse Practitioner in St. Louis, MO making upward to $160,000

Our innovative care model is integrated into the communities of our patients and focuses on the quality of care rather than the volume of services.
Salary & Benefits:
- Salary Range: $105,000 - $135,000 (Experienced providers with bonuses can earn $150,000 - $160,000)
- Annual Clinical Performance Bonus:
- Less than 5 years of experience: $15,000
- 5+ years of experience: $25,000
- Full Health Benefits
- Reimbursement for Mileage and Toll Fees
Role: Transitional Health (Home Health)
- Focus: Post-discharge visits, including:
- Medication reconciliation
- Social service follow-ups
- General follow-up care and care coordination
- Caseload: 6-7 patients per day
- Support: Medical Director available for guidance
Required Qualifications:
- Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program graduate
- National certification in at least one of the following specialties:
- Family Nurse Practitioner
- Adult-Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US work authorization
