Location: Philadelphia, Pennsylvania
$118k - $122k + Full Benefits
Clinical Magnet, part of ICON Medical, is hiring a Nurse Practitioner to join our partner's Institutional Special Needs Plan (ISNP) team. This full-time, permanent position is based in skilled nursing facilities where you'll deliver comprehensive, patient-centered care to an older adult population.

What you'll do:
Perform comprehensive health assessments and create individualized care plans
Collaborate with the member's primary care physician and interdisciplinary staff
Diagnose and manage both acute and chronic conditions
Order and interpret diagnostic tests, prescribe medications, and make referrals as needed
Act as the clinical liaison, ensuring seamless communication and transitions of care
Support patients and families while serving as a trusted resource to the care team
What we're looking for:
Current Nurse Practitioner license (Acute, Adult/Gerontological, or Family NP)
At least 3 years of clinical experience (SNF/LTC experience preferred)
Strong commitment to caring for the elderly population
DEA/State prescriptive authority (or eligibility)
Basic Life Support (BLS) certification
Valid driver's license and reliable transportation
