Join a collaborative outpatient care team that includes physicians, nurse practitioners/physician assistants, medical assistants, dedicated scribes, nurses, and care managers. Our integrated network of specialists and hospitals allows our providers to focus on what matters most—exceptional patient-centered care.
Job Responsibilities
Conduct annual wellness exams and health risk assessments with detailed, holistic documentation.
Coordinate care with specialists, hospitals, and community resources.
Lead population health initiatives (e.g., preventive screenings and chronic care management).
Support phone triage, patient outreach, and health education.
Participate in community and promotional activities.
Conduct home visits as needed.
Qualifications
Board certified in Family or Adult-Gerontology Primary Care.
Active, unrestricted state NP license and DEA registration.
Eligible to work in the U.S.
