Home Health APP- FNP or PA
Location: Seattle, Washington
Start Date: As Soon as Credentialed – Ongoing (Must commit to 6 months)
Schedule: M-F 8a-5p (8-hour guarantee)
Minimum shifts/coverage to be considered: 4 shifts per week, prefers 5.
Typical procedures required: Primary Care, Wellness Exams, Chronic disease, population health, and Kidney care.
Patients/day: (Typical 3-4) (3-6 if in proximity)
Ages: Adult patients
Support Staff: At-Home RN’s + LCSW’s
Credentialing/Onboarding Timeframe: 90-120 days
(Can credential in less than 90 if enrolled with Medicare/Medicaid/MediCal)
Orientation: National office 3-day paid travel (Brentwood, TN)
