Job Title:Nurse Practitioner
Location: Leavittsburg, OH 44430
Shift: Monday–Friday (No evenings, weekends, or holidays)
Duration: Approximately 3 months (with potential for extension)
Hours/Week: 40
Job Responsibilities:
• Monitor and manage patients with chronic illnesses in accordance with facility policies and medical protocols.
• Administer immunizations and vaccinations; prescribe medications or provide treatment for acute and chronic conditions; perform minor procedures as training and licensure allow.
• Conduct routine physical examinations to assess, diagnose, and manage chronic and new medical conditions.
• Evaluate the need for specialty consultations (internal or external) and review consultation reports, approving or disapproving recommendations as appropriate.
• Order and review diagnostic tests such as laboratory, radiology, and other medical assessments.
• Collaborate with medical leadership to complete medical reclassifications and requests for non-formulary medications.
• Provide both routine and emergency medical care for assigned patients.
• Examine, diagnose, and treat patients within the scope of practice as defined by the State of Ohio Nursing Board.
• Conduct rounds on infirmary patients as needed.
• Document all patient encounters, assessments, treatment plans, and follow-up in accordance with facility guidelines and healthcare standards.
• Participate in quality assurance activities, accept guidance and feedback from supervising medical leadership, and comply with established protocols.
Qualifications & Skills:
• Current Nurse Practitioner license in the State of Ohio.
• Minimum of one year of experience as a Nurse Practitioner in primary care or a related field.
• Board Certification (Family or Adult-Gerontology NP preferred).
• Current CPR/BLS certification for healthcare professionals
• DEA certification.
• Ability to work effectively in a correctional or secure healthcare environment.
