Job description We are seeking a Family Nurse Practitioner for a 120-day locum tenens opportunity in Arizona starting Dec 29, 2025. This assignment involves day shifts, Monday through Friday, in an outpatient primary care clinic within a hospital. Candidates should have at least 2 years of chronic care experience, with a strong emphasis on diabetes management.

Responsibilities and Duties
  • Deliver primary care services in an outpatient clinic setting within the hospital
  • Manage chronic conditions with a focus on diabetes care
  • Provide patient assessments, develop treatment plans, and monitor progress
  • Collaborate with interdisciplinary team members to ensure high-quality patient care
  • Document all patient encounters using the RPMS electronic medical record system

Additional Information
  • Minimum 2 years of chronic care experience required
  • Experience with diabetes management is important
  • IHS experience preferred but not required
  • Must hold Family Nurse Practitioner Certification (FNP)
  • Required certifications: Advanced Cardiac Life Support (ACLS), Basic Life Support (BLS), Drug Enforcement Administration (DEA)
  • Licensure: Any state license accepted
