Responsibilities and Duties

- Provide patient assessments, develop treatment plans, and monitor progress
- Collaborate with interdisciplinary team members to ensure high-quality patient care
- Document all patient encounters using the RPMS electronic medical record system
Additional Information
- Minimum 2 years of chronic care experience required
- Experience with diabetes management is important
- IHS experience preferred but not required
- Must hold Family Nurse Practitioner Certification (FNP)
- Required certifications: Advanced Cardiac Life Support (ACLS), Basic Life Support (BLS), Drug Enforcement Administration (DEA)
- Licensure: Any state license accepted
