Requirements:
• Active CA License
• BC
• BLS
• DEA
• Current CPR
• With at least 3 yrs of experience in a similar setting
• PAs must have a CA Controlled Substance Education Course Certificate
• NPs must have a Furnishing License with Schedules II
• Immunizations such as Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer, etc.
