Certified-NP for Home Health Assessments. 5 - 6 patients per day.
Urgent Need: As soon as credentialed or by October 15 - end of year
Schedule: 8 hour days, 5 days a week (M-F or W-F example)
Hours of operation 8 AM - 8 PM (8 hour days, this includes chart and travel time)
Scope of Practice/Responsibility:
Visit Type A. Level 2–3 Diagnostic Visits — close quality gaps for health plans:
- A1c, bone density, KED, FIT test, diabetic retinal exam, etc.
- Perform diagnostics, document results, follow-up steps
Visit Type B. Level 4 Comprehensive Home Annual — longer visit:
- Full annual wellness + labs/diagnostics
- Assess Social Determinants of Health; document needs/recommend resources
- Care Guides convert signed charts into a 30-day community resource for the member
Visit Type C. Telehealth Annual Wellness — Limited availability for locums.
Job Responsibilities
Comfortable using iPad for charting.
Chronic disease assessment, vital intake, labs etc.
Qualifications
Valid Drivers' License
Licensure: Michigan NP with active MI RN (standard MI rule)
Payer credentials: Medicare & Medicaid enrollment required
Not accepted: No PAs
Working Hours
Start date: October 15
Credentialing start: October 1 (fast-track; offers within 24 hours after interviews)
Initial term: Through end of calendar year (extensions likely)
Holiday weeks: No visits Thu/Fri/Sat/Sun of those weeks
