Primary Care Nurse Practitioner (Spanish) - Osceola county, FL
Job Type: Full-time (On-site)
Schedule: Monday-Friday, 7:30 am - 5:00 pm; on-call responsibilities alternate between providers with a call center prescreen
Responsibilities:

Conduct comprehensive patient assessments, including history, physical exams, and diagnostic evaluations.

Develop and implement individualized care plans, including prescribing medications within scope of practice.

Order and interpret laboratory and imaging tests; refer patients to specialists as needed.

Maintain accurate, detailed patient records using the EHR system (ECW).

Provide patient counseling on lifestyle, diet, medications, and treatment options.

Participate in quality improvement initiatives, including HEDIS and MRA compliance.

Collaborate with physicians and other healthcare providers to manage complex cases.

Supervise and coordinate with medical assistants and support staff for regulatory compliance.
Qualifications:

APRN/NP license required; at least 2 years of experience in General Practice, Internal Medicine, or Family Medicine.

Spanish language proficiency required.

Experience with patient-centered care, chronic disease management, and preventative care.

Caseload: Approximately 17 patients per day (follow-up/new patient ratio once established).
