About the opportunity:
- Setting: In-home visits to health plan members (primarily 65+, can include 18+ Medicaid)
- Start date: October 15
- Credentialing start: October 1
- Coverage needs: Through end of calendar year (extensions likely)
- Holiday weeks: No visits Thu/Fri/Sat/Sun of those weeks
- Schedule: 5 days per week, prefer M - F but open to any 5 days (W - Sun, etc.)
- Hours: 8-hour day (includes drive time + charting); available window 8a–8p, will be 8 hours within this window
- Daily volume: Target 5–6/day, maybe more
- Certification: FNP-C (or GNP) — must be board certified in Family Practice, Internal Medicine, or Geriatrics.
- Experience: New grads considered if relevant RN experience; strong preference for in-home comfort, 1:1 patient interaction, and experience with screening/diagnostic workflows
Job Responsibilities
Perform and document diagnostic tests and results, including recommended follow-up steps.
Complete comprehensive annual home wellness visits, including labs and diagnostics.
Assess and document Social Determinants of Health during visits and identify patient needs.
Recommend appropriate community or health plan resources based on assessment findings.
Qualifications
