Timeline

  • Start date: October 15
  • Credentialing start: October 1 (fast-track; offers within 24 hours after interviews)
  • Initial term: Through end of calendar year (extensions likely)
  • Holiday weeks: No visits Thu/Fri/Sat/Sun of those weeks

Work & Scope

  • Setting: In-home visits to health plan members (primarily 65+, can include 18+ Medicaid)
  • Daily volume: Target 5–6/day (may schedule up to 8 to buffer cancellations)
  • Hours: 8-hour day (includes drive time + charting); available window 8a–8p, will be 8 hours within this window
  • Schedule: 5 days per week, prefer M - F but open to any 5 days (W - Sun, etc.)
  • Independence: Visits conducted solo; Check-In Specialist handles member check-in

Visit Types (three tiers)

  1. Telehealth Annual Wellness (phone) — limited use for locums
  2. Level 2–3 Diagnostic Visits — close quality gaps for health plans:
  3. A1c, bone density, KED, FIT test, diabetic retinal exam, etc.
  4. Perform diagnostics, document results, follow-up steps
  5. Level 4 Comprehensive Home Annual — longer visit:
  6. Full annual wellness + labs/diagnostics
  7. Assess Social Determinants of Health; document needs/recommend resources
  8. Care Guides convert signed charts into a 30-day community resource “Porter Plan” for the member

Credentials & Requirements

  • Licensure: Michigan NP with active MI RN (standard MI rule)
  • Certification: FNP-C (or GNP) — must be board certified in Primary Care
  • Payer credentials: Medicare & Medicaid enrollment required
  • Experience: New grads considered if relevant RN experience; strong preference for in-home comfort, 1:1 patient interaction, and experience with screening/diagnostic workflows
  • Tools: Comfortable with iPad/tech
  • Safety: If unsafe, leave and notify practice coordinator
  • Not accepted: No PAs

Equipment & Logistics

  • Provided: Medical equipment for visits (not clothing)
  • Travel: Flight in/out; hotel via corporate account
  • Ground: Rental car/gas or mileage reimbursed
