We are working on an exciting new opportunity with a growing healthcare company that is seeking Nurse Practitioners or Physician Assistants in San Fernando, CA to work visiting patients at home and virtually. -Full time schedule in patient homes and at home: Monday through Friday 8:30 - 5:00 pm
-Home care or hospice experience preferred
-Chronic disease management experience preferred
-Must be willing to travel within the designated market, up to 1 hour from home
-Must be willing to visit patients in their homes 4 days/week, 1 day remote
Pay flexible
*For this location, there will be opportunity for travel. Typically 1x a month and could involve driving 2 hours (maybe more) away, staying overnight, or spending up to a week conducting visits.
