We are working on an exciting new opportunity with a growing healthcare company that is seeking Nurse Practitioners or Physician Assistants in Seattle, WA to work visiting patients at home and virtually. There could be opportunity for travel to outlying areas across the state. Travel is on a case-by-case basis. We would love to share more with you about this opportunity. -Full time schedule in patient homes and at home: Monday through Friday 8:30 - 5:00 pm
-Home care or hospice experience preferred
-Chronic disease management experience preferred
-Must be willing to travel up to one hour from home
-Must be willing to visit patients in their homes 4 days/week, 1 day remote
