Barton is seeking a skilled Functional Medicine Nurse Practitioner for a telehealth assignment. The position starts immediately, with a schedule of 3 days per week, and potential for full time extension. Working hours are typically 8:00 AM to 5:00 PM. The NP will focus on hormone replacement therapy, thyroid optimization, weight loss, metabolic support, and various wellness-focused consultations. Responsibilities include conducting video consultations, interpreting advanced labs, developing personalized care plans, and managing patient follow-ups through the CHARM/CANVAS EMR system.

Provider Type Needed: Functional Medicine Nurse Practitioner (Telehealth)


Schedule: 3 days per week, including Mondays and Fridays with potential for full-time extension



Responsibilities and Duties:


• Conduct initial consultations and new patient lab reviews via video


• Review and interpret advanced labs (GI-MAP, Methyl Detox, ALCAT, hormone panels)


• Develop personalized care plans including lifestyle, nutrition, and supplementation


• Manage asynchronous follow-up care via EMR platform


• Educate patients on test results and coach on health optimization


• Complete all documentation in EMR (CHARM/CANVAS) within 24 hours


• Provide concise, functional medicine-informed SOAP notes


• Communicate with support staff to coordinate care


• Prescribe and manage bioidentical hormones, peptides, and nutraceuticals


• Monitor patient response and adjust dosing as needed


• Collaborate with compounding pharmacies and support teams



Additional Information:


• Active NP License in good standing needed


• 2+ years of clinical experience required


• Strong communication skills for effective virtual consultations


• Ability to work independently and as part of a multidisciplinary team
