PRN Nurse Practitioners – In-Home & Facility-Based Health Assessments

Are you a compassionate and self-driven Nurse Practitioner looking for independent, meaningful work that fits into YOUR lifestyle?

We’re hiring Board-Certified NPs across the state to perform Comprehensive Health Assessments (CHAs) for Medicare, Medicaid, and commercial members in their homes or long-term care settings.

🕐 Work on YOUR terms:

✅ PRN Role – minimum of 20 visits per month

✅ Choose your own schedule – full or half days

✅ Visits scheduled a month in advance

✅ Work close to home (60-mile radius)

✅ Get paid even for no-shows ($25 each)

🩺 Your Day-to-Day Will Include:

🔹 Conducting full non-invasive physical assessments (CHAs)

🔹 Visiting patients in homes, SNFs, and assisted living facilities

🔹 Identifying urgent/emergent care needs and escalating as appropriate

🔹 Communicating findings with PCPs and internal teams

🔹 Using a company-provided tablet to complete and submit documentation

🔹 Traveling within your assigned 60-mile radius – reimbursed mileage included

🧰 What We Provide:

✔️ $110 per completed assessment (Depending on your state)

✔️ $25 per no-show

✔️ Mileage reimbursement

✔️ Paid training + onboarding

✔️ All tech & PPE provided, including:

 • Samsung Galaxy Tablet

 • Lab kits for in-home screening

 • PAD sensors (for certain populations)

 • Gloves, goggles, sanitizer, masks, thermometer, etc.

 • Door hangers and leave-behind patient flyers

🩺 Requirements to Apply:

✔️ Active CA license

✔️ Board Certified – FNP, AGNP, ENP, or ACNP preferred

✔️ Strong clinical decision-making and documentation skills

✔️ Tech-savvy – comfortable using tablets for charting

✔️ Willingness to travel (mileage may exceed 120 miles/day)

✔️ Reliable personal transportation

✔️ Must provide your own clinical tools:

 • Manual BP cuffs (3 sizes)

 • Clinical-grade scale (≥ 300 lbs)

 • Measuring tape, pulse ox, stethoscope, pen light, monofilament
