Job description

We are looking for a nurse practitioner (NP) or physician assistant (PA) to join our dedicated post-acute care team in Manhattan, New York.

Position Details:

  • Full-time, 5 days per week, Monday through Friday

  • Patient volume averaging 17 patients per day

  • Post-acute care setting serving short-term and long-term patients

Key Responsibilities:

  • Provide primary care services including chronic disease management and illness diagnosis

  • Develop and implement individualized care plans

  • Order and interpret laboratory tests

  • Prescribe and monitor medications

  • Collaborate with physicians and coordinate care with healthcare teams

  • Educate patients and caregivers on managing chronic and age-related conditions

  • Build strong relationships with patients, families, and facility staff to ensure effective treatment delivery

  • Promote healthy lifestyle choices

Qualifications:

  • Minimum of 1 year of primary care experience as an NP or PA

  • Post-acute care experience preferred but not required

  • Active and unrestricted New York NP or PA license

  • Active and unrestricted DEA with prescriptive authority
