Nurse Practitioner - Upstate New York | Multiple Locations

We’re hiring primary care nurse practitioners for multiple post-acute care opportunities throughout Upstate New York.

What to Expect:

• Location: Multiple sites throughout Upstate New York

• Schedule: Flexible weekday rounding (part-time and full-time options available)

• Setting: Skilled nursing and post-acute care facilities

• Role Type: W2, benefits-eligible

What You’ll Do:

• Deliver primary care to patients in post-acute settings

• Manage both chronic and acute conditions with a patient-centered approach

• Collaborate closely with interdisciplinary teams to improve patient outcomes

• Build trusted relationships with patients, families, and facility staff

• Partner with a supervising physician while working with autonomy

Who We’re Looking For:

• Licensed and certified nurse practitioners (FNP, AGNP, CNP, APRN)

• Active New York license (or ability to obtain)

• Compassionate, team-oriented, and motivated clinicians

• Post-acute or long-term care experience is a plus, but not required
