Nurse Practitioner - Farmington, New Mexico

We’re hiring a primary care nurse practitioner (NP) or physician assistant (PA) to join our post-acute care team. This flexible position offers a customizable schedule, autonomy in your practice, and the opportunity to make a meaningful impact in the lives of patients.

Position Details:

• Location: Farmington, New Mexico

• Schedule: Flexible full-time, 3.5 to 5 days per week

• Setting: Post-acute care inpatient facilities

• Call: Weekday, daytime on-call (Monday through Friday)

Key Responsibilities:

• Provide comprehensive primary care services

• Manage both acute and chronic conditions

• Develop individualized care plans

• Collaborate closely with the interdisciplinary care team (IDT)

• Work in partnership with a supervising physician

Ideal Candidate:

• Nurse practitioners (FNP, CNP, AGNP, APRN) or physician assistants

• Holds an active New Mexico license

• DEA license preferred (not required)

• Open to both new graduates and experienced clinicians
