- Hospital-employed opportunity for an experienced Nurse Practitioner or Physician Associate to join a collaborative team
- Competitive salary plus incentive potential

- Join a multidisciplinary team including primary care providers, mental health professionals, a care coordinator, and support staff
- Full-time schedule: 34 patient contact hours over a 4.5-day workweek
- Minimal phone-only call
- Must have prior experience in a primary care setting

- EPIC EMR system
- Approximately 2–2.5 hours to Portland and 1.5–2 hours to Eugene with access to major airports nearby
