Job description
We are looking for a Board-Certified Family Practice Nurse Practitioner for a 13-week locum assignment. This role involves conducting home-based health assessments across Ocean County, NJ. You will review patients’ chronic conditions and complete risk assessments in a supportive and independent setting. Driving is required — mileage will be reimbursed daily.

Job Responsibilities
Review chronic conditions and conduct A1C tests as applicable
Identify urgent findings and escalate to PCPs as needed
Document all visits using iPad-based EMR
Maintain professional communication with patients and care teams
Qualifications
Board Certified as Family Practice NP
Strong interpersonal and communication skills
Prior home health or outpatient experience preferred
Ability to manage a daily patient visit schedule independently
Working Hours
8-hour shifts per day scheduled in advance
No weekends or on-call required
Skills
Tech-savvy with strong EMR skills
Ability to work independently in the field
Compassionate bedside manner and strong rapport-building skills
