- 36 Patient Hours/Week, 4 admin time, four-day work week available
- 16-20ppd, APRN's have their own panels
- EMR: EPIC, AI scribe service imbedded
- Academic/Clinical Affiliate of Yale University School of Medicine

- 20 days PTO, 6 paid holidays, 5 sick days, 5 days CME w/stipend
- Base Salary $120K-$148K (based on experience), generous sign-on bonus, wRVU bonus plan, Quality Bonuses, retirement plans, relocation expenses, student loan reimbursement

Job Responsibilities
- Conduct comprehensive health assessments, including obtaining detailed medical histories, performing physical examinations, and ordering/interpreting diagnostic tests.
- Develop and implement individualized treatment plans in collaboration with the supervising physician, focusing on health promotion, disease prevention, and management of acute and chronic conditions.
- Prescribe medications, order therapies, and manage medical devices according to state regulations and collaborative practice agreements.
- Provide patient education and counseling regarding health maintenance, disease management, medication adherence, and lifestyle modifications.
- Evaluate and manage common acute illnesses (e.g., upper respiratory infections, influenza) and chronic conditions (e.g., hypertension, diabetes, asthma).
- Perform minor office procedures as trained and within scope of practice.
- Collaborate effectively with physicians, nurses, medical assistants, and other members of the healthcare team to ensure coordinated and comprehensive patient care.
- Document all patient encounters accurately and thoroughly in the electronic health record.
- Participate in quality improvement initiatives and adhere to all organizational policies and procedures.
- Serve as a resource for medical staff and participate in educational activities as needed.
- Provide age-appropriate care to patients across the lifespan, from infants to geriatrics.
- Manage referrals to specialists and coordinate follow-up care.
- Maintain current knowledge of family medicine best practices and engage in ongoing professional development.
Qualifications
- Active PA or NP in family medicine specialty certification.
- Active, unrestricted CT medical license or in process of obtaining
- ACLS/BLS/PALS (as required) Certification
- DEA License
