Job description

Faith-based Federally Qualified Health Center seeking Advanced Practitioners (NPs or PAs) to provide quality, comprehensive healthcare to the homeless and underserved. They provide full spectrum primary care to adults in a faith- based setting, including MAT, HIV and Hep C treatment, and have strong integration with dental, mental health and optometry care, as well as care coordination and housing navigation. Practice is an outpatient only setting with no call.

Job Responsibilities

Under supervision of staff physician, practitioners will perform evaluation and treatment of patients in the outpatient setting sites. Assess medical needs and compile care plan, addressing acute needs, chronic conditions, and preventive health maintenance. Assessing needs may include assessment of spiritual, emotional, mental and social needs. Furnish/transmit appropriate medications and treatments. Perform in-office procedures such as incision and drainage, arthrocentesis, nail removal, skin biopsy, and cryotherapy.

In addition, they have a contracted Psychiatrist providing part-time Telehealth Psychiatry care to improve patient’s access to psychiatric care. There are 3-4 primary care teams serving at this site, and they also provide pediatric care and women’s health care at this site. They do not currently provide prenatal or OB care but are open to adding this service.
