Advanced Practice Provider (APP) – Transitional Care Team
An Advanced Practice Provider (APP) is needed in Philadelphia, PA.

Salary and Benefits
- Base Salary: $105,000 – $135,000
- Bonus: Performance-based annual bonuses (up to $25,000 for seasoned providers; $15,000 for those with <5 years' experience)

Responsibilities
- Conduct post-discharge home visits as part of the Transitional Care Team
- Perform medication reconciliation, assess social determinants of health, and ensure continuity of care
- Coordinate with primary care and specialty providers for follow-up services
- Manage a caseload of approximately 6–7 patients per day
- Utilize EMR to document visits and communicate with care team
- Engage in quality improvement initiatives to enhance patient outcomes
Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program
- Board Certification as one of the following:
- Family Nurse Practitioner (FNP)
- Adult-Gerontology Primary Care NP
- Adult NP
- Gerontological NP
- Active and unrestricted NP license in the state of Pennsylvania
- Active DEA license
- Minimum 2 years' experience in primary care or home health with a geriatric focus preferred
- Prior experience with post-discharge or transitional care is a strong plus
- Must have US work authorization
