🌟 Home Health Nurse Practitioner – Full Time Permanent Opportunity in Conroe, TX🌟
🩺 About the Role
Position:
- Location(s): Conroe, TX
- Specialty: NP treating patients with CKD and polychronic conditions
- Support: Will be supported by team of RNs and SWs
- Schedule: M-F 8a – 5p; 1-2 days working remote; 3-4 days treating patients in-home
- Full-time, permanent position
- EMR: Athena
Requirements:
- FNP, AGNP, or ACAGNP
- 2 years of direct patient care NP experience
- Clean and active TX state license
- DEA license
- BLS
