Are you a seasoned Nurse Practitioner passionate about relationship-based care for older adults? A leading value-based primary care group is launching services at a new senior living facility in the Greater Boston area and is seeking an experienced NP to lead clinical operations on-site.

Position Highlights:
- Serve as the primary care provider for a new senior living community
- Deliver thoughtful, unrushed care to a stable panel of older adult patients
- See 8–10 patients/day in a supportive, low-volume setting, average just over 1 visit per hour
- Benefit from virtual care team support (social workers, nurse care managers, community health workers)
- Collaborate with engaged physician leadership and a nationally recognized ACO partner
- Enjoy flexible scheduling options
- Board-certified NP (FNP, ANP, AGNP)
- Clinical experience, ideally including geriatrics, assisted living, or home-based care
- Comfortable with clinical autonomy and proactive care delivery
- Familiarity with value-based care and Athena EHR (or willingness to learn)
