Premise Health is seeking a skilled and compassionate Nurse Practitioner for a locum tenens role at their onsite clinic located at Roche Molecular in Santa Clara, CA. This is a premium W2 opportunity with a focus on employee health and wellness in a low-volume, supportive setting.
📍 Position Details:
- Location: Roche Molecular, 2821 Scott Blvd., Santa Clara, CA 95050
- Schedule: Monday–Wednesday, 8:00 AM–4:30 PM
- Start Date: June 30, 2025
- End Date: August 27, 2025 (with potential extension if no internal hire is made)
- On-Call: None
- Setting: Onsite employee health clinic
✅ Requirements:
- Certifications:
- Board Certified (ANCC or AANP)
- DEA
- BLS
- Licensure: Active California NP license required
- Experience:
- Minimum 1 year of experience in Primary Care or Occupational Medicine
💼 Role Responsibilities:
- Provide primary and occupational health services to employees
- Manage acute and chronic conditions
- Collaborate with clinic staff and support team
- Use Epic EMR for documentation and communication
💰 Compensation & Perks:
- Rate: $80/hour (W2, all-inclusive)
- Support: Full clinic support team
- Orientation: 2–3 days
- Onboarding: 2–3 weeks
