Our innovative care model is centered in our patients' communities, focusing on the quality of care over the volume of services.

Salary & Benefits:

• Salary Range: $105,000 - $135,000
• Experienced providers with bonuses can earn $150,000 - $160,000
• Annual Clinical Performance Bonus:
  • NPs with less than 5 years of experience: $15,000
  • NPs with 5+ years of experience: $25,000
• Reimbursement for Mileage and Toll Fees

Role: Home Health Care
Each NP or APP will be supported by a dedicated care team:

• Care Team: Medical Assistant, Scribe, and a designated Social Worker
• Visits: Average 6-7 visits per day
• Supported by the Medical Director, who will serve as the NP/APP's mentor
• NP/APP will conduct the same workups as in a clinical visit

Required Qualifications:

• Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program graduate
• National certification in at least one of the following specialties:
  • Family Nurse Practitioner
  • Adult-Gerontology Primary Care Nurse Practitioner
  • Adult Nurse Practitioner
  • Gerontological Nurse Practitioner
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization
