CleanSlate Centers is a nationwide medical group providing office-based outpatient treatment for the America’s crisis of addiction. We are currently looking for a Nurse Practitioner for our outpatient substance abuse treatment center located in S. Louisville, KY! This position entails the initiation and maintenance of medication assisted treatment.
