CVS Health and Oak Street Health are building a new way of delivering senior primary care with a value-based personalized approach designed to deliver the right care at the right time – by removing barriers and gaps to patient care. We are expanding our house calls team in the New York City market ,and based on your profile, you might be a great fit!
The House Calls NP delivers high-touch, high-quality, home-based primary care to the highest-risk patients. Supported by a Medical Assistant and Scribe, NPs will make 4-6 visits daily in a defined territory and collaborate continuously with other team members for care coordination, social services, and behavioral health care.
