Family Medicine APP *120535: Northern, New Hampshire
Duties: recording physical and mental status, performing physical exams, ordering diagnostic tests, implementing and managing plans of care, as well as prescribing and dispensing medications and more.
Provider must have minimum of 2 years in practice in outpatient primary care, rural health clinic experience preferred, able to work autonomously
Must be Board Certified
Direct patient care time will be split between acute care and established patient panel.
Demographic: Pediatric through Geriatric, High elderly population
Experienced in Chronic Disease Management
Volume: 18-24 patients per day
36 patient hours, 4hrs of admin time
No call
EMR: Meditech Expanse, Dragon dictation
Supported by an RN Team leader and medical assistant
