Nurse Practitioner or Physician Assistant (Primary care)
Location: Salinas, CA
Job Type: Full-time
Shift: Monday - Friday, 8 am- 5 pm
Work Location: In person.
Pay Range: $130k-$160k
About Your Work:
- Obtain history and perform physical examination
- Complete review of medicines and related instructions
- Review home safety and psychosocial needs
- Fulfill documentation needs for STAR and HCC measures
- Obtain and document all pertinent information in patient's charts and case management notes on the same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions.
Qualifications:
- CA Licensed Nurse Practitioner
- 1+ Experience with Primary Care
